How to Get Started with a Remote Consultation

In order to expedite your new patient on-boarding process, we ask that you send the following materials. This process will decrease the time and effort required to obtain previous medical records from other physicians, which will allow us to complete your initial lab order much more quickly. The sooner we can get your lab order completed and your labs drawn, the faster your new patient on-boarding process can proceed. Our office must receive the completed new patient packet and all supportive documents before your lab order is completed.

These documents include:

  • Deposit Agreement & Payment Policies (emailed for e-signature immediately after your initial consultation with the doctor)
  • New Patient Packet (emailed for e-signature after your e-signed Deposit Agreement & Payment Policies has been received and payment processed successfully)
  • Laboratory Results (within the past 1-3 years, as available, emailed as PDF attachments or faxed at the same time you complete the New Patient Packet)
  • Pathology Reports (if surgery or biopsy has been performed, emailed as PDF attachments or faxed at the same time you complete the New Patient Packet)
  • Imaging Reports (X-ray Radiographs, CT scans, MRI scans, Ultrasounds, etc.; emailed as PDF attachments or faxed at the same time you complete the New Patient Packet)

Once we have received all of your documents, you will enter our new patient queue. Once you reach the top of the queue, the doctor will complete your initial lab orders and email them to you for fulfillment at your local laboratory patient service centers. Once all of your initial labwork is final, the office will call or text you to schedule your lab review visit with the doctor. Dr. Partovi generally spends at least as much time reviewing your medical records and ordering your initial labwork as you have scheduled for the initial lab review.

If your initial consultation does not involve discussion about a previously diagnosed medical condition, but you have specific questions you would like to discuss during the consultation, feel free to email or fax your questions to us at least 24 hours before your consultation for Dr. Partovi’s review. Please include your name and scheduled consultation time with the transmission.

Our remote services are not a substitute for a visit to your primary care physician. Consultations complement and enhance the care you are receiving from your conventional local doctor.
